Create a reusable information form
Write your questions once in Custom Forms, then attach the form to any request or template.
Who it is for: Owners, admins and members of a UMNIII Verify Workspace.
Keep in mind
- A form collects information supplied by the recipient. An answer is not independently verified because it was collected through Verify.
- Where you need something independently verified, add a separate supported check.
- Forms can contain personal information. Ask only for information you have a lawful and legitimate reason to collect.
Step 1: Find Custom Forms
- Open Custom Forms from the menu in UMNIII Verify. Owners, admins and members can create, edit and archive forms; viewers can read and preview them.
- If building your own forms is not switched on for your Workspace, the forms UMNIII provides are still available when you compose a request.
- A Workspace may have a limit on the number of forms; the page says when it is reached.
Step 2: Add, label and order the questions
- Press “New form” and enter a “Form name” and, if you like, a description.
- Press “Add a question”. Write the question in “Label”, choose its “Type” from the kinds of question your Workspace may ask, and add help text where it helps.
- For a list, write the choices with “Add a choice”. For a declaration or consent, a tick box can be set so that it must be ticked to submit.
- Recipients answer in the order shown. Use “Move up” and “Move down” to reorder, and “Remove” to take a question out.
Step 3: Decide what is required
- Tick “An answer is required” for a question that must be answered, and leave it unticked for an optional one.
Step 4: Preview and save
- Press “Preview as the recipient” to see the form as your client will. Nothing typed in the preview is saved or sent.
- Press “Create form” to save it, or “Save changes” when editing.
Step 5: Make it available, and share it if you choose
- A saved form is available to your Workspace straight away, under “Your forms” when a request is composed.
- Owners and admins can also publish a form to the UMNIII Verify shared library, where other Workspaces can preview it and adopt a separate copy. Only the questions are shared, never clients, requests or answers.
Step 6: Use it in a workflow
- In the composer, “Send verification invite”, tick “Collect information (forms)” and choose your form under “Your forms”.
- Press “Save as template” to keep the combination, so the same form is attached every time.
Step 7: Find the answers
- Answers are filed against the verification request. Open it from Verifications to see them under “Submitted Forms”; “Evidence and downloads” has the downloadable report and evidence pack.
Step 8: Reuse it, edit it, retire it
- One form can be attached to any number of requests and templates.
- Editing a form changes it for future requests. A request already sent keeps the questions it was sent with, so past answers stay exactly as filed.
- “Duplicate” makes a variant without touching the original, and “Archive” stops a form being offered without deleting anything.
